2022 Annual Awards Banquet - August 31, 2022Renaissance Orlando at SeaWorld It’s that time of year again to recognize the people who work so hard to make a difference in our world of Transportation Disadvantaged services. Please take this opportunity to nominate individuals, teams and organizations who have gone above and beyond in contributing their time and/or expertise to the Transportation Disadvantaged program. We have provided a nomination form for each category that incorporates a questionnaire to be completed. There is still space for you to express yourself in a narrative format, but now there is specific data that will be used by the Awards Committee to compare the different nominations. Please take the time to answer each question. If you cannot answer a question, please state that you “don’t have or don’t collect the information” or, when appropriate, state it’s “not applicable”. Please make sure to review the Awards Criteria (included on the Nomination Form) to ensure you have all the required backup documentation prior to your submission. Do not include copies of training certificates or employee recognition documentation with your nominations. You may summarize this information in your narrative or support letter. Please send a digital photo of your nominee. We will need this photo for the awards presentation. To review the criteria and make a nomination. Forms are available here. These are Word-fillable forms, with a box to type in for each question. If the document does not allow you to enter information, click on ‘View’ tab in the upper left corner of your screen, then select ‘Edit Document’. This should change the form so you can enter information. Also, use your keyboard ‘Tab’ key to move from one box to the next. The deadline for submitting Award Nominations is July 8, 2022. Nominations can be emailed to CTDOmbudsman@dot.state.fl.us or mailed to the address listed on the form. Awards will be presented during the CTD Awards Banquet which will be held on Wednesday, August 31, 2022, in Orlando, Florida.
